Image Rectification

What is the 2D projective transformation that transfers the vanishing points into infinity points so that I get a rectified fronto-parallel image?
Let u=[ux, uy] and v=[vx, vy] be the two vanishing points, please tell me the homography that do the task.
It may be quite easy but I am not able to find it for so long
